If he won't wear the cone of shame or socks, I suggest that you get him a greyhound muzzle. The large plastic basket kind. They seem to be comfortable to wear and it will stop him from licking at his feet.

If he can't lick, then athletes foot medication will get most fungus infections. You don't want to have him eating it. If it is bacterial and your vet is charging you so much money, then I assume that he has cultured and is giving you the correct antibiotic.

I question whether or not removing toes will work. It seems to me that he would simply move on to the next toe in line and start working on that one.

I second the suggestion to investigate food allergies. Common allergies might be caused by cleaning products, too. Carpet cleaner is a big one.

Also, antidepressant medications will sometimes help. Consult your vet.
