The vast majority of hospitals have a charitable program for folks who can’t pay. I’m NOT talking about a payment plan. It’s sliding scale, based on income. The hospital will absorb a considerable percentage of the bill depending on the persons income.
Don’t assume his income won’t qualify! You might be very pleasantly surprised!
How it works:
They will bill insurance (if you have, such as medicare or HMO, whatever) and you wait for the hospital bill to come. Once you have bill in hand, call the billing office at the hospital, that you would like to apply for charity assistance. They’ll send you a form.

I have used the charitable hospital here when my appendix was removed.
