laying nest...how many?

I'm getting ready to build my nest boxes inside my coop. 3 questions!

1. how many hens do I figure per nest box?

2. what dimensions do I use for each nest box

3. how big should the entrance way be into the nest box?
 
I Just use old milk crates and cut half of one side out.
I have four but my 8 hens use only one.
Mum kept complaining about the noise from them when they layed.As they line up waiting for the same box.
So i put a golf ball in each one of them now they use all of them and now theres not much noise.
Go for around 1 box for 4 chooks
 
Most say a nestbox for each 3-4 hens. However, if you have a small flock, say only 4 hens, I'd stick 2 boxes in. You never know when a hen could go broody and hog a box for a month.
