Lethargic Hen

Please help! Today I found one of my girls very lethargic. She has a slimy substance emitting from her vent. As I am fairly new to raising chickens, I really need some help as to what this may be and what to do. Any help is much appreciated.

So, I think that I found the issue. While going over our yard with a fine tooth comb it looks like she laid a soft shell egg that may have broken while she tried to lay it. She is no longer leaking any fluid, however she is still lethargic and barely eating. Any advice as to she I should do at this point?
