Lethargic, smells like urine, purple comb. . . HELP!

I have a sick hen who is lethargic, moves very slowly with a bit of a waddle, her rear end is rather low to the ground as well. She also smells like urine and none of the other chickens smell this way. Her comb is purplish. I saw her eat yesterday. She is two years old, free range and has been sleeping separate from the rest of the chickens. Can anyone help me diagnose and treat her?
 
Kalmbach crumble, Scratch and Peck mash, black soldier larve, bird seed. She also has pasty butt, green and white color. I haven't seen her in the nesting boxes for a long time so I'm assuming she's not laying currently.
 
The urine odor is probably from the white urates. Is her enlarged lower belly firm or does it feel like fluid? I would try to get her drinking fluids, electrolytes or sugar water, and offer some watery chicken feed and cooked egg. She may be dehydrated or having some reproductive issues. Heart failure could also be possible.
 
No pin feathers. Gave her a soak in epsom salt and cleaned her bottom. I found a worm crawling in her feathers, a tiny white one that was a little longer than a grain of rice. Can I give her Ivermectin (horse paste)?

Are you sure that was not a maggot? Can you make sure there are none around her vent or on a wound? Pictures can be helpful.
