When I lived in Atlanta I had a chicken with heart disease that was treated at the UGA vet school, exotics department. They were wonderful.

I just wanted to second this recommendation for Medical Center for Birds. Ours had some unusual problems and while it's a drive to get there, they are far and away the best chicken vets in the area. Dr. Speer has written several avian books, and has 11 backyard chickens of his own.
