Live Hatching Problem..... Need Seasoned Hatchers HELP

I am not new at all hatching live or incubated chicks. This year for the first time ever both of my live hatchings have resulted in ALL of the chicks fully formed but dying before they are hatched fully. Any suggestions??? my hen is in a full hay box, has access to water and food no differently from before???

hmm.. first thing i would do is clean the coop, maybe theres bacteria killing them? what are you feeding the hens? i've noticed from a personal experience, i gave one of my hens terramycin so i thought well since i cant eat them i'll incubate, and everyone one of her eggs made it till around day 18 fully formed but never hatched, im new to all this also, im sure someone knows exactly what signs to look for and what to do!

I would think the same reasons would cause the same problems whether hatching under a hen or in a bator.
