my hen has lost her ballance

she is an ex batt hen and i am not sure of her age at all. she has seemed a little tired lately,over about three weeks, i put it down to the extreme heat we have had. yesterday she was stood next to me in the garden, stood up but falling asleep. she has done this many times before. then she fell over to her left, it was funny at first but when she didnt bother to get up i could see she was not really all there. today after work she was not out and about, infact she hadn't moved all day. i helped her to her feet and she collapsed. could any body please tell me if she is not long for this world, or if there is something i can do for her? many thanks
