Need help quick

have a 3 yr old Narragansett tom who is not doing well. Thought it was gapeworm, not responding to treatment.
Symptoms are swollen snood, lethargic, extending his neck often, standing upright (for lack of a better term), darker head, not gobbling, watery droppings
Please help
