One hen laying egg with balloon like shell

I have 1 out of 8 hens which for some time has layed a egg that breaks into the nest.
She looks mostly healthy, tail up, nice comb. She does have a lack of feathers on neck front and forward of vent. Vent is dirty. Although this has been happening for a while, I have just discovered who is the culpret.
Looking for best advice to treat or deal with problem.

Yes there is always calcium on the side.

I just bathed her. cleaned out vent area. Put her in the broody cage with food and water (with ACV and save-a chick electrolyte).

Checked bath water for worms. The only thing I see that could possibly be a worm is this. 6mm long. Thanks

I would cut that open to see more.

Soft shelled eggs are usually because of lack of calcium or a dysfunctional reproductive system. They can also happen with a young new layer and in older birds before and after molting time.
How old is this bird?
What all and how exactly are you feeding?
