Organ? Hanging out of sak, not the vent

Black astralorp 3 yrs old. Sak had has been the not bothering her for a over a year. Today was letting another hen eat from her. There was blood not a ton. She's calm. She had been laying eats and doing normal chicken things. I'm in Milwaukee if anyone is local. What is it? What can I do?

I would take her to vet immediately. Are there any vets that take chickens near you?
