our poor chick

We have three baby chicks we got from a friend who had chicks at about almost 2 weeks old to put with our broody hen. It all went so well and was just lovely to watch she loves her babies. All was fine till this evening when we found one chick dead in the coop. She was just fine today running around like a maniac getting to the food before anyone else happy with worms from my kale plants. What can cause a baby chick to die in the span of 3.5 hours?

It could've been that the hen stepped on it.

really? i can't see that happening because they are a bit big and they are still sleeping under her. are there diseases I should be cautious of?
