Particles and oddly colored yolk

I've been getting a few eggs like this. I know it is coming from one of my two Rhode Island Reds. Is there something wrong? It had been happening infrequently, however over the last couple of weeks we've been seeing it whenever she lays. The yolk also is not of consistent color.

We did find one of our RIR dead about 2 weeks ago. She was fine the day before and she had no injuries, it looked like she just fell off the roost and died. She was 2 years old. All of our hens appear to happy and healthy.
