Yes, be concerned.
What I do: Make the hole larger so you know the chick can breathe. Make sure you peel back all of the membranes around the chicks nose/nostrils. Leave it be for a while but check every so often and make sure it's still breathing ok. Don't rush opening the shell. Every hour or so after that, I chip away at the shell if the chick is making no progress. Very slowly do I help the chick out. You can't just go in and chip it out quickly. the blood vesssels need to slowly close off. It could bleed to death if the hatch is too fast.
Be very careful you don't shrink wrap the chick by doing this.
If you don't help it, it has a good chance of dying because the aircell is not in that end of the egg.
