Polish Hen Lethargic/Eating Very Little

My darling hen, Effie, is having a really hard time.Yesterday she was very lethargic and would only eat out of my hand, and today she's worse. She's still eating, but not much. Her crop is empty. Her droppings are an yellowy-olive color, fairly watery. She's also limping on one leg.

I've brought her into the basement (dad wouldn't let me keep her in my room), and she has a warm little dog crate with straw and food/water.

She's a decent layer, however the last three days I haven't gotten anything from her. She'll be a year old next month (May 20th's her hatchday).

She seems to be very nervous, but extremely tired. She'll only go to sleep if I'm holding or petting her.

I tried feeding her strawberry tops, and she ate one vigorously but then didn't show any interest in the rest.

I intend on sleeping by her cage tonight, hoping she'll calm down and get some good rest.

She is the lowest in the pecking order (6 hens total), and although not mistreated she doesn't have any friends. She's usually a very flighty and active bird...we like to joke she's part Roadrunner. Right now she isn't talking at all and is quite content with anything we do to her.

Please help me figure out what to do! She's a beloved pet in our family, and after losing two silkies I loved very much this past month due to predators I don't think I could personally take her leaving us.
 
Have you done a bug check? Have you wormed them lately? How do you think she got the limp? Could she have gotten attacked by something?
 
Yes, I've done a thorough check of her feathers. I don't see any exterior pests, but don't know about interior. She was vaccinated for Marek's as a chick (we bought her off of mypetchicken.com), but don't know about any other possibilities.
The limp is almost certainly an old injury. She got attacked by a fox and luckily escaped with nothing but a hurt foot about a month ago, but made a full recovery.

Today, her droppings were nothing but liquid. I force-fed her some yogurt (she was so weak she couldn't protest). She can't support her own weight, let alone walk. She's been trying to talk but nothing comes out but a faint wheeze. Constantly sleeping, barely responds when I approach her. :(

Should I give her electrolytes? Take her to a vet?
 
She needs to go to a vet.
