Poop Burns Hen's Rear

I have a Plymouth Rock that has a problem of her poop not falling clear and clinging to her feathers. Sometimes it builds up quite a bit before it falls off. This last time when I looked she showed signs of having loose bowels as her rear end was covered with dark watery poop, and maggots were in it. I tried to clean her up and get the stuff off her and it looks like her skin is eaten away in a large patch. I cleaned the maggots and got all I could off her then swabbed her down with neosporen really thick. She is a fat though a bit lethargic but eats fairly well. She quit laying regularly about a year ago and lays only rarely. I just gave the girls some cooked grits and two scrambled eggs and she joined in.

What is wrong with her and is there anything I can do to help?
