After researching the import/export regulations, I found this link. Hopefully this will be helpful to you. At the bottom of this page, it links to each individual state's requirements about importing poultry and eggs into the respective state. It looks like almost every state requires an NPIP certified flock of origin - some states also require an accompanying state health certificate form.
