Routine Flock Testing.... Resources?

Hi All,
Who/what lab do you use flock testing - for things like Mycoplasma and other serum tests? Do you take samples yourself and send them out, or do you have a vet do it, or do you take samples and run tests yourself? I'd like to do a test every 6mo to annually to monitor my chickens (for peace of mind). I'd love any recourses you may have to get me started. I also check fecals routinely through my vet for worms, but I'd appreciate any resources you have that could do all the testing for me in one fell swoop.
