i worry i had something to do with it, before she passed i had her on her back like a baby, because she was so weak, then i had her on her back when i was carrying her upstairs because her neck went limp. maybe she choked on crop fluid after being on her back

I highly recommend you send her body for a necropsy.
Some of the most common reproductive orders can be prevented by what you feed your bird.
