Scalped chicken/bloody vent

Hi so I unfortunately trusted my chicken was healed enough to go back into the coop with the flock. Of course I checked on her a hundred times and everyone seemed fine.
Today unfortunately she is scalped and her vent in extremely bloody. She is obviously separated again and I have sprayed wound wash on her but I am at a loss for words and incredibly mad at myself and my other chickens. Any advice is greatly appreciated. She is eating a little bit of the eggs I made her and her vent doesn’t look prolapsed so far from what I can see.
If I need to add pictures I can try but I honestly can barely look at her because it looks so bad and I am not good with blood.
 
Pictures would be very beneficial. Keeping it clean to prevent infection from setting it is very important. How long was separated from the flock ?
In my experience pulling a chicken for injury or sickness any longer then 4 days I almost always have to a reintroduction. Usually takes another 4 days to a week. I just keep them in a separate pen that everyone can see them. Then I'll let them out with everyone and watch. Sometimes a little fighting other times nothing. It doesn't take long for a chickens to forget a flock mate. I had an older hen I brought in for one night because it was very cold and she was molting. I made the mistake of bringing her out of the house after I'd let everyone else out. They all went nuts. Had to bring her back in. So next morning I brought her out 1st then let everyone out and fine 🤷🏻‍♀️ I also have 4 coops and around 70 ish chickens. It can be a circus sometimes
 
Poor thing. Don't beat yourself up. Couple questions:
Why was she separated in the first place? Injury/illness?
How long was she separated for?
She was injured from a hawk attack I have a previous post from it. She was put back into the flock for about a day and a half before this. I checked yesterday and she was fine so I am not sure. They did not even peck her original wound! They pecked her head and vent area. She was originally separated for about two days inside before rejoining the flock.

She was only separated for about two days before rejoining the flock. She is the only bantam silkie and was attacked by a hawk so I am not sure if they knew she was just weak and smaller. I should have checked more frequently but everything seemed fine. Like I stated they didnt even go after her original wound

I'd keep her separated for a while now because the bright red will attract an attack again. Once she's completely healed and no longer weak, you'll have to do a "see but no touch" intigration as if she's completely new to the flock.

I posted pictures I think below. But thank you so so much for the kind words. They just survived an incredibly harsh winter and then a hawk attack so I am feeling incredibly defeated. Especially because we were going to hatch some of her eggs this spring

Poor thing. Sometimes we can feel defeated when raising animals. It seems like things always go wrong back to back and you just feel helpless. Hang in there! Hopefully the joy outweighs the difficult times.
