Sick Hen!! Please Help!!!!!!!!

I have a sick hen who is about a year old. She has been sitting in one spot and she keeps her eyes closed all of the time. Her neck kinda sways too. She is eating but we have to hold the food right in front of her. She will not eat on her own. She shivers a lot and we have her in a cage with a towel under a heat lamp. She is getting thinner and she hasn't laid any eggs in over a week. Please help!!!!!!!!!!!!!!!!!!!!!!!! We have been feeding her layer ration and we have been crushing up berries to give her more energy. Does anybody know what could be wrong with her?
