Soft Spot on Egg

I have one chicken that regularly has been laying an egg with a soft spot and hole on the narrow end. What causes this. Not see before.

Thanks for this article. She is a young hen and has 5 "sisters" All are doing just fine on the diet which they have been on for a year. I guess I'll have to separate her and monitor. I don't think she is pecking at it. The "spot" is soft and has a little hole in it.
