Swollen Tom Neck

We have a tom who has a deformed beak. Oddly enough he has managed to figure out a way to ear. Today, we found him with a swollen neck. It has no air in it. His head is tilted. We did manage to put his head straight but he did not like that (seems it hurt him). It is dark so I can`t go back and take photos.
Three weeks ago, it was swollen at the same place and it went away.
Any idea?

If it was painful to move the neck, do you see any bruising or injury?

Is he eating well and getting enough nutrition?

Take a look at his beak since has a deformed beak to make sure it's not getting caked up with debris or clogged up with mucous, etc.

It wouldn't hurt to give him poultry vitamins to see if those help.
