What ingredients do I need to make one?.... I have the blue and cuckoo parts....

That's all you need.
cuckoo male with two barred genes bred to blue hen will give you 50/50 single barred/non barred and 50/50 blue/black chicks.
the pullets that are barred and blue are finished product.
the males with barring will need to be bred to a barred female and half of those male offspring will be double gene barred. when you do the second cross just use a blue male or blue female to continue producing blue chicks.
every time you cross blue and black you'll get 50% blue.
