Have you guys checked the show all link on the notification drop down? If you miss a reply it wont notify you.

It's because I get a bajillion notifications and if I miss clicking it one time in my notifications/alerts then I don't get anymore.

I think you can turn offthe reaction alerts. Not sure if its thread specific though.
