Update:Thanks for the help everyone but she is gone

So I have a hen with diarrhea that is weak and a little thin and I don't know what to do. She is almost 6 years old and started showing symptoms a couple of days ago.
I have been giving her antibiotic water and electrolyte water and she has been eating a little cottage cheese. As of this morning she has had a yogurt/water/protein water mixture because she stopped eating.
She is tired and sleepy this morning and not looking great. She doesn't have mites and was mite powdered a week or two ago. Her breathing sounds normal. She has a heat lamp and slept on a perch all of last night.
Is there anything I can do to save her? Do you think she has worms? We have not wormed the flock in a year but have wormer. We are planning on worming the flock soon.
Thanks in advance I love her and any advice helps.

I am sorry she is sick.
What antibiotic are you using in her water?

How hot is it under that heat lamp?

What worm medicine do you have on hand?
We have Wazine 17 worm medicine on hand. The antibiotics are called Duramycin-10 and I think it is the only kind we have. The heat lamp is not very hot, it is probably only 60-70 degrees it is for a tortoise. I gave it to her because she was shivering a lot. Thanks for the quick reply.

I'd have to say stop using the antibiotic...it is not a cure all.
I hate to say this but she has stopped eating which to me is not a good sign.

I would definitely worm her but not with Wazine. Wazine only gets roundworms.
BUT with her being so sick I'm not sure that is a good idea.

Is getting her poop checked by a vet an option?
Are you willing to tube feed her?

Getting her poop checked by the vet isn't really an option, I wish it was though. I am willing to tube feed her, what do you recommend feeding her? Are you sure I should stop using the antibiotic? Could the wormer be too much if I gave it to her?

If you were to offer her some complete chicken feed mixed with some water and and egg or tuna do you think she would eat it?

Cottage cheese sounds disgusting (TO ME) I wouldn't eat it either! LOL
I'm thinking that tuna fish might smell a little better???
