Third illness in same chicken over a year. Please help!

Could her scab just be a reaction from prior poop build-up?

I wouldn't think so. She usually has a pretty clean butt, and I give them a wash if I see a mess.

I don't think her butt area looks too bad though...
I'm guessing that white build up is just poop. I don't know what else to tell you.

I would first make sure she has the strength to survive illness.
* Feed meat protein: boiled chicken, beef or pork. You can give tofu, but meat works better.
* Feed black or red grapes for energy, moisture, antioxidants.
* Moistened (water or aloe vera juice) layer mash free choice.
* Vitamin supplement. I like Vionate sprinkled on feed.
* keep her warm not hot.

Then, a fecal float test ASAP. You need to know what is weakening her body. A blood test would be more accurate in diagnosing, but I know sometimes money is an issue. A CBC is an expensive test, $100.

You have taken very good care of your hen for her to live to five years old. God bless your little hen and peace to you.
