Two hens always fighting

I have a polish and an orpington (once best friends) that have now seemed to develop a vendetta against each other. All they seem to do is fight! It has gotten to the point where they can no longer be together. The polish gets head feathers pulled out, and the orpington got her comb ripped off. They have lived together for 3 years, and this behavior came out of no where. Should I continue to keep them apart? or will this never solve the issue? Can this be fixed? Here is a quick video that shows them trying to fight even through the chicken wiring! Please let me know any tips to fix this! thanks.
 
I always solve for the peace of the flock, fighting upsets all the birds, and can even cause laying to be suspended or slowed down. I would cull one bird, but if the second bird finds a new sparing partner, I would cull that one too. Sometimes I have read that one bird can sense something wrong with the victim. Sometimes true, sometimes not. I think older chickens can develop dementia, and to me a 3 year old bird is an older bird.

No changes in flock population like new chick/birds and/or loss of a flock member?
