vitamins & electrolytes

I bought the Durvet V & E product to feed to my flock but can't figure out the correct dosage. Does anyone have any suggestions for mixing this in the drinking water?

I do one quarter of a teaspoon per GALLON - not that much, but the dosage is on here somewhere..............
