Since your chicken has been on antibiotics, that can cause diarrhea because of the killing off of good bacteria in the gut. I agree with getting a fecal sample checked for things such as coccidiosis, worms, and enteritis. But I would be using probiotics daily, then every other day. Those help build up the good bacteria in the gut.
 
Capillary worm treatment requires several days of dosing with something like Fenbendazole (Safeguard, liquid goats or paste for horses). The dose I use is 50mg/kg by mouth for 5 days, which is .5ml of either the liquid or paste per 2.2 pounds.
