What caused her death halfway through unzipping?

So this is my first time hatching and I have to say, it was very successful. 13/14 are hatched and healthy but one didn’t make it. Based on the photo, can you tell what caused her death? I’ve seen on here before where people are able to tell what went wrong. She was pipping towards the bottom so not sure if that caused it.

Seems like it was zipping just fine, so it's probably not that it's the wrong end. Agree on the liquid as mentioned above. Or did you add oil to moisture it?
There's also blood so it could have hit a vein by accident and bled out. They don't have a huge quantity of blood in their tiny bodies, so they can't lose a lot before it becomes critical.
