What causes this?

In the last couple of months, I have had 2 or 3 of my hens start bleeding from their vent. I thought at first that it was prolapse but after it happened now to this many, I am wondering if there is something else going on and if it is not prolapse. Once the other hens find out that that hen is bleeding, they peck the crap out of it because they go for the blood, so I separate them but then, they never make it. They make it for a long while and then I think that they get really egg bound and then die. Has anyone else experienced this? What is causing this? Is there something that I can do for it? I am going to treat them for coccidia if anyone thinks that that is what it is. Will that take care of all of it or does this not have anything to do with coccidia?

How many hens/roosters do you have and how much space? (Sqft of coop and run)
What do you feed including treats?
There are no roosters with these hens, and I would say the coop is about 24 square feet. There are 12 hens in it now. I feed a poultry feed mix from a feed mill that is powdered. They sometimes get oyster shells and I have given them the corn and water froze as treats since it is so hot right now through summer. I have also given them meal worms before and like a seed mix.
 
How much run space is there?

12hens in a 24sq ft coop is very close quarters. This can lead to quite a bit of squabbling and picking.
Likely when the hens are laying eggs, another hen is picking at their vent. Once blood is drawn, the others join in. Chickens like blood and they will pick another to death.

Can you provide more space? If not, then this behavior could continue and you lose more birds.
Placing curtains over nesting areas may help a little.

I hope you find some solutions to your problems. Giving them more space can help. If that's not possible, then consider reducing the number of hens you have. I know that might be a hard thing to do, but if they are injuring each other, then it's time to make a few changes.
