I just noticed this when I checked on my hens tonight. Any help is appreciated!

This is the 1st you have noticed it? Is it blood from the vent?
Yes. We checked on her again this evening and it looked a little better. The blood had clotted and there was clear mucus coming out like she was flushing it out.
 
Nope no other concerns other than the fact that she is in with 14 other hens and one of them was laying double yolks at random times maybe once a month.
 
How old is she and does she lay?
She is about 3 and yes she does lay. There was a regular sized egg that she layed that had blood on it which is when she started bleeding I am guessing. It also crossed my mind that she may have been the one that was laying double yolks every once in a while but the last time I got a double yolk was a week and a half ago to two weeks ago.
 
Your hen looks like she was pecked and cannibalized by the other chickens. Sometimes they can have a prolapsed vent that is red and attracts pecking. Sometimes it is because of overcrowding and boredom. Once she is cleaned up with a soak in warm soapy water or Epsom salts water, take a closeup of her vent. If she has a prolapse, the red tissue needs to be kept moist with Neosporin, oil or vaseline until you can push it back inside. Here is a good article about prolapses:
