What's the most common cause of newborn chick death?

It seems like once my chicks make it past 3-4 days, they're good to go. But I often have some
that are very lethargic and "sleepy", not open their eyes much at all. They usually don't make it
past day 2. I know there are dozens of different illnesses/diseases, but is there a very common
one in new born chicks?
 
The two most common mistakes that I see when multiple chicks are dying in the brooder are...

1) Brooder too warm
2) Lack of enough fresh air/ventilation

Depending on how the brooder is constructed, getting away from the light doesn't necessarily get them away from the heat.
