Wheezing and black spots

I have a geriatric hen that is wheezing and developed black spots on her comb. She was wheezing also about 6 months ago so I treated her for a respiratory infection and with antibiotics and electrolytes she got better. The black spots are new this round. Like before I’ve isolated her from the others and have been treating her again but she isn’t responding this time. Any suggestions??
 
Maybe there’s a blockage in the trachea causing her to lose oxygen? It also possibly could be from a heart issue, such as congestive heart failure or heart disease.

It was hard to get a good photo but the back of the comb is also turning dark which may just indicate the condition she’s in.
