swap Chinese hole in the wall for buffet. Any place with the word "buffet" associated with it is one sneeze away from a food poisoning epidemic.
Nah, I've got a good hibachi, hole in the wall, and fancy buffet here. The only way you'll get sick is if you stuff yourself or eat the sushi on the buffet.

People get pretty lax about sanitizing stuff or keeping meats hot or cold enough.
I've seen some Olive Garden employees try to walk out of the bathroom without washing their hands. Thankfully a loud "OH MY GOD YOU ALMOST JUST FORGOT TO WASH YOUR FILTHY NASTY HANDS MA'AM!!! GOOD THING I WAS HERE TO REMIND YOU! WE ALL GET FORGETFUL SOMETIMES I GUESS." makes them halt in their unsanitary steps and walk back to the sink🤣

Not enough enforcement mostly. Health departments and federal agencies responsible for inspecting and managing food are habitually understaffed and inspections don't get carried out as often as they should. Compound that with very little in terms of penalties for violations and no incentive to follow best practice, and voila! germ salad in your salad. In AZ it takes a LOT to get shut down for health code violations. Even if food-borne illness has been traced to your restaurant. And since chains are typically run as franchises or corporate entities, you're more likely to see bad practices there becuase it's someone else's money / problem if the health inspector finds violations. Sole proprietorships generally have more at stake from getting shut down so they do better (not always but on average).
