It has come a long ways. There was a time when smoking was allowed in hospitals and doctors offices. It was very common to walk in and see nurses and doctors sitting at a nurses desk smoking. Patients smoking in bed. People didn’t think anything of it. Now, it seems ludicrous. The prosecution rests, your honor.
It's not as bad as it was mid 20th century (say 20-60's). Smoking is not glamorized like it was back then. and depending on what part of the country you're in it's not as prevalant. Most places have indoor bans on smoking, smokers lounges are more or less a thing of the past, etc... It's moving the right way, but it's going to take time.
What is normal is just whatever the majority of the population thinks is normal or are made to believe is normal by the powers that be which usually is the government. The powers that be have more tools to work with at their disposal than ever before.
There are a few things that are in the process of being made to be normal now that not too long ago were considered abnormal just like smoking is being pushed in the other direction.
