The temperature here this morning was 19 degrees.... When i went out to open up the coop I noticed the coop ( smaller prefab from TS) was covered in frost.... could this happen from lack of ventilation or just that it was extremely cold?

Yes, that would be not enough ventilation.

It was on the outside....the inside temperature seemed to be just a little warmer than the outside temp but not by much

I've never seen a pre-fab coop that had adequate ventilation. If you didn't add (a lot of) ventilation, my money is on not enough ventilation.

Temps and humidity should be about the same inside and outside the coop.
If not then you need more ventilation.
