Worm infestation??

Yes, the SafeGuard in the feed is usually not given to chickens as a wormer, since it is hard to know how much they need to get the right dosage. Reproductive disorders can also cause walking like a penguin. Have you tried to give her a dose of calcium or Tums orally, incase she has a stuck egg or lash material? That would not hurt.
She might be eggbound, try soaking her in a container with warm water and follow the instructions in post #2 in this link:

Thank you guys so much for your suggestions. I would have tried that, but sadly she has passed away this morning. I am so sad.
 
Sorry for your loss. With her upright position, I suspect she had salpingitis and egg yolk peritonitis. The yellow urates in the droppings are common with that. If you still have her body, and would like to do a home necropsy to take a look at her organs and take pictures, that could help us to give you some opinions. Post any pictures here.
