Wrinkled eggs to thin shelled eggs to no eggs

Hi. I have been searching trying to figure out if my hen has an illness. I've read about the symptoms but never together. She is an amber white, about 14 months, had been great layer. Then shell color got lighter & larger with more pointy tips. Then tops became wrinkled (over a month) then noticed a few body check eggs. Then a thin/weak shelled egg that she didn't even go into the hen house to lay. She had a few more hard shelled eggs but really large & abnormal shaped with bulge on side. Now nothing unless she is dropping them where I can't find. She goes into hen house to lay, but never leaves an egg. She shows no other odd behaviors. She has always had some issues with brown streak down butt but a couple mornings it was really nasty. She has some normal poos, but a few have been really runny/watery with some stringy strands. No worms from what I've seen. Other 3 hens have no issues. One has always laid shell-less eggs, I figured was organ defect. Free range with lots of earthworms lately. ACV in water & on layer feed. Get scratch, meal worms, BOSS, yogurt & cheese on occasion. No nasal or eye discharge. I'm concerned but not sure what exactly to do. Dewormer can't hurt, suggestions on type/brand would be appreciated? Any suggestions on what if any illness I should be concerned about & treatment? Thank you!
 
Just yesterday I caught her just sitting & sleeping in the yard. Not active or scratching around. Then she squirted out yolk & egg white. Still acted uncomfortable and she had a few more squirts of this stuff. Hubby & I gave her bath in Epsom salt & some vitamins. No prolapse or other issues detected while bathing & handling. She pretty much just went straight to bed & definitely hadn't eaten as much as usual before bed. Her routine in the hen house for bedtime was a little off.

This morning I found a soft shell with powdery build up that she must have pushed out over night. Seems to be fine so far today. I'm just stumped as to why the change in egg shell & all these issues she's been having. I don't want her to be in pain or uncomfortable.

It's now been well over a week since her last hard shell egg. I'm starting her on duramyacin 10 to prevent infection & giving vitamin water. Any one think the progressively deteriorating eggs could be from worms or any other issues I should be fighting. I'm just stumped & want to help her badly.
