Wry neck getting better, then worse, then better, then worse again?!? Help!!!

I have a 4 month old satin rooster named Harley. On 2/24 we noticed signs of wry neck. We started him on vit e caps. On 2/28 I went out to the coop to find Harley flailing on the ground, unable to straighten his neck. When he stood, his neck was under his belly (6:00 on the clock) We drove into town and got vit e paste and red cell from Tractor Supply. By 3/1 in the afternoon he was keeping his head in the correct position about 70% of the time. He would twist to the side primarily when I moved him or he was off balance. He was getting a pea Suze amount of vit e 3x per day and 1cc red cell 1x a day. In the AM on 3/2 he had back slid and unable to bring his head up. His head was stuck in a 3:00 or 9:00 position. He was still eating and drinking on his own. On 3/4 we went back to town (it's a long distance fir us) and got him b complex vitamins. We started giving him b complex twice a day along with vit e. We upped his red cell to 2cc a day. He started slowly getting better. On 3/7 he was keeping his neck up about 80% of the time. At 630 this morning, 3/8, he was holding his neck up fine and crowed for the first time. When my husband came down at 830 he was laying down. When I got home at 5pm his neck was under his belly again (6:00 position). He has been falling over, freaking out, and getting stuck on his back. He was also laying on his belly instead of on his feet. Right now he is sleeping with his legs splayed behind him. He has been shaking his head more than usual and he smells worse than he has been recently, despite having a bath on Sunday.

Up until this point he had been eating food (mixed with a lot of water) on his own as well as eating veggies, seeds and grass. He isn't eating, even when topped with mealworms. I dipped his beak in water and it's almost like he was falling asleep in the water before taking a sip. I tried this for 10min before I switched to a syringe.

Why is he fluctuating so much? At what point is it better to end his suffering. I'm new to chickens so have never been in this position. I have 3 hens without any issues and 3 recently rehomed roosters that also are fine.

I'm sorry he's not getting better.
If you have video or photos of him that may be helpful.

What do you feed? (type of food)

Wry Neck is a symptom. Vitamin therapy may help depending on the cause. It's used a supportive care to see if symptoms will improve.
A common cause of neurological symptoms like Wry Neck is Marek's disease. I would consider this since he's laying with his legs splayed behind him. Some birds do respond and can "get better" even with Marek's disease, but they will be carriers for life.

Looking at what you are giving, I would be concerned that he's getting too much Selenium. If giving the E+Selenium paste along with Red Cell likely that's too much.
Generally 400IU Vitamin E and 1/4 tablet B-Complex along with some egg is sufficient.

You mention he smells worse - is it because he's soiling himself or is the odor something else? Infection? Sour Crop? Yeasty Vent Gleet?

You can try placing him in a sling, but if he's flailing or having seizures, you will want to monitor him closely so he doesn't get hurt.
