Is her tail up? Have those black spots always been on her comb? Is she preening as normal?

That dosage does seem really high. We have an 8 pound hen who was on clavamox after bumblefoot surgery. She was taking 125mg twice a day.

Here is a comparison. The photo on the left is from the day we found her (a week after the attack). The right photo is from a couple week ago. About 2 months in between the photos.

Also, some beaks grow back, some do not. I think our chicken's beak is growing back, but at a snail's pace.

The top beak looks different -- almost chipped (but maybe I'm just imagining it?). Was there any injury? Maybe there's a lack of blood flow? Just a guess. It is very strange.
