I had a hen doing that and I gave her VetRX for Poultry. Got it at TSC or Rural King. I put a little in her nostrils and rubbed some on the top of her head. The next day it cleared up.

I sorry you lost your hen. It sounds like you did everything you could to save her. I also have a sick chicken and can't find any answers for her condition.
