1.5 Yr Hen loosing weight??

I have 1.5 year old Light Brahma hen. I treated them for coccidiosis a few weeks ago. Haven't changed anything in their diet. I treated her and the other chickens for worms the day I realized she was looking rough and felt really light. I gave them 1.6CC of liquid goat wormer. I gave her scratch grains with her pellets along with some clover and grass for some natural vitamins.

Any thoughts on what his could be exactly?
