Its not a great picture but try to hold a 5 week chick still.
Doesn't seem to be blood but looks like a scratch with just red skin around it. I have no idea what happened or what could have caused it. Do I need to do anything??? It doesn't seem to be bothering her at all either but its just something I noticed this morning when I let them outside for the day

Can you maybe try to get another picture? Sorry, I can't really tell anything from the bottom one at all.
 
It's kind of below that round thing on the side of the chickens heads, right above her neck.

Will try for a better picture but she doesn't like being picked up.
 
What ever it is, it doesn't look bad enough in those pics to worry about. If it is a cut, make sure it has stopped bleeding. You can put a dab of triple antibiotic on it, but your bigger problem is going to be preventing the others from picking at the wound. I would watch for that more than anything. You can put a dab of Blu-Kote on it to both disguise it and for its antiseptic value. You're not supposed to use Blu-Kote on food-producing animals, so you will have to make your own decision about that.

In the updated pics, it looks like feather picking--assuming I am looking at the right thing. The neck is a common target. Same thing applies as stated in my last post.
