Abnormal poop for 4 months now

My hen is 4 years old and she has had very abnormal poops for 4 months now. I can’t seem to get down to the bottom of what’s going on with her. Here is a picture of what it almost ALWAYS looks like. Please help!!!!! I don’t know what else to do.
