She is lethargic - has she been laying eggs?
What does her poop look like?
Do you have a rooster?
Do you notice new feathers coming in on the rest of her body?
Is her crop empty in the morning before she eats/drinks?

I agree, the feather loss on the back could be from mating, picking or possibly molting.
The "lethargy" is the most concerning. Check her really well for lice/mites while you are checking to see if she has any bugs.
