The item numbers have B in front of them because it's Form B, there are two equivalent "forms" of the test with different item sets, Form A and Form B. The file I linked is for Form B.
So for item 12 on Form B, which is labeled as B12, you'd select your response by indicating response 1, 2, 3, 4, 5, or 6, next to the item number. 1 is furthest left while 6 is furthest right. So 12/B12 - 1 if you wanted select response 1 for item 12, or 13 - 1 if you wanted to select response one for item 13.
