Bantam Chick Hatching Issues

I got her all the way out of the egg about an hour ago. There were no blood vessels attached to her and she had full absorbed her yolk sack, so she was basically just sitting in the egg exhausted and stuck because it was too dry.

Do you have any idea what may have caused so much of the shell to damage when she tried to hatch? It happened to another egg of hers from this hatch that I wasn't able to save. The hen that was sitting on the eggs hardly moved at all so I know she didn't trample the egg or anything so I'm just trying to figure out why this happened to try and avoid it in the future.

Was she turned the wrong way in the shell? Could be mama tried to help it out.
