Broken toe with bone sticking out

I appears from the photo that the flesh and skin from the toe has been stripped away from the bone, and the bone is split lengthwise.

I would soak the foot well in Epsom salt, then take a close look. If the bone is hanging and not attached, I would remove it. Any remaining flesh should be dressed with antibiotic ointment and Vetericyn, then bandaged to keep out dirt. Any damaged flesh will eventually blacken and fall away. The hen should be able to get around okay without the toe.

If this also happened previously, I doubt it was a car. Maybe a dog or other animal. If it was from a broken fence, it would be wise to locate the cause of injury and remedy it.
